Home Health Aides
Home Health Aides help people who are disabled, chronically ill or cognitively impaired, as well as elderly adults who may need assistance living in their own homes or in residential facilities. Most Home Health Aides work with physically or mentally disabled patients who need more care than family or friends can provide.
Home Health Aides provide support in the assistance of daily life activities (ADLs), including but not limited to:
- Light housekeeping
- Changing bed linens
- Shopping for food
- Planning and preparing meals
- Helping get out of bed
- Bathing assistance
- Dressing assistance
- Personal care/grooming
- Accompanying to doctors’ appointments or other errands
Thanks to modern medicine, seniors are living well into their 80s and 90s. Many have active lives and are capable of maintaining their independence with a little help and supervision. Our Companions provide non-medical services to a elderly people who are living alone and are at risk for accidents and mental stress due to isolation and loneliness. Companions make it possible for seniors to remain in their homes and maintain their independence with dignity.
Our Companion services include, but not limited to:
- Assistance with routines and the activities of daily living such as bathing, grooming and dressing
- Mental stimulation
- Escorting for walks
- Meal planning and preparation
- Medication reminders
- Shopping and running of errands
- Light housekeeping and laundry
- Changing bed linens
- Driving and/or accompanying to the physician when necessary
- Shopping and performing other household errands
- Supervising ambulation and helping to prevent falls
- Providing socialization, friendship and support
Nutritional counseling services consist of an assessment of a patient’s nutritional status and any recommendations for dietary changes. Our Registered Dietitians can counsel the patient directly or provide consultation and direction to the nurse case manager. We also provide this service to non-symptomatic patients wishing to maintain and preserve a healthy lifestyle.
Physical Therapy services are offered to individuals who demonstrate functional limitations. A Physical Therapist’s role is to provide skilled intervention to improve the patient’s level of functioning in self-care, mobility, safety, strength or range of motion and to establish a safe and effective maintenance program. Examples of therapy services include assessment of need and development of an appropriate rehabilitation program for the patient.
